Chronic disease is now the dominant health burden


The National Health and Morbidity Survey (2023) shows just how widespread chronic disease has become in Malaysia:

  • 15.6% of adults have diabetes, nearly 3.6 million people, one of the highest rates in the world

  • 29.2% have hypertension, roughly 1 in 3 adults

  • 33.3% have high cholesterol

  • Over 2 million adults live with three of these conditions at once; half a million live with all four


These conditions are managed continuously, often for years — through medication, monitoring, and lifestyle support — rather than resolved in a single visit. Nearly 2 in 5 people with diabetes don't yet know they have it, which makes early detection just as important as ongoing treatment.


Chronic disease management


This is where chronic disease management (CDM) comes in — a coordinated, ongoing approach to supporting members with these conditions, rather than treating each visit as a standalone event. A strong CDM approach typically includes:

  • Risk registers, not just claims history. Proactively identifying members with or trending toward a chronic condition using claims and screening data, rather than waiting for the next hospitalisation to reveal it.

  • A dedicated care coordinator. TPA as a single point of contact who manages a member's journey across screening, referral, and follow-up, instead of leaving coordination to the member.

  • Structured, time-bound programmes. Defined lifestyle and coaching programmes for at-risk or newly diagnosed members, run in partnership with clinical or digital health providers, rather than one-off advice.

  • Clear referral pathways within the network. Defined escalation rules from GP to specialist within the provider panel, so members move smoothly through their care journey rather than navigating it alone.


Why this matters for employers, insurers, and members

  • Chronic disease is a major driver of long-term healthcare costs and lost productivity. Supporting members earlier and more consistently helps manage both.

  • CDM gives visibility into member health earlier in the journey, rather than only reviewing outcomes after a claim is filed.

  • Better coordination means clearer guidance, fewer gaps between visits, and support that continues beyond a single treatment.
